The authors find a suggestion of "an inverse association between high fluoride exposure and children’s intelligence." However, note that the "high fluoride exposure" is naturally occurring in the available drinking water and was not added by an authority of some kind, and note also that the levels were as much as 11.5 mg/L of water(page 14), while US standards limit fluoride to 0.7-1.2 mg/L of water -- the levels were much higher than US standards. (The high levels of naturally occurring fluoride are in China, by the way -- not the US.) Based on the results of their review of other studies, the authors state that "the results tended to support the potential for fluoride-mediated developmental neurotoxicity at relatively high levels of exposure in some studies." (Pages 13-14). They also note that the Chinese drinking water may contain arsenic, lead, and iodine at neurotoxic levels.

So my understanding from the actual study is that the authors' review of other studies may tend to support the potential for neurotoxicity when the fluoride is at high levels (about 10 times US levels). The authors also note that the estimated decrease in IQ is "within the measurement error of IQ testing" (Page 14) and that "the actual exposures of the individual children are not known." (Page 15) The authors also point out that "each of the articles reviewed had deficiencies, in some cases rather serious, which limit the conclusions that can be drawn." (Page 15) The authors also say, "the studies were generally of insufficient quality." (Page 16)

TL;DR -- the study reported on is a review of other research articles over 10 years where the water studied had as much as 10 times the US limits, the research occurred in China, and the articles reviewed had deficiencies; this leads the authors to conclude that there may be some evidence of toxicity of fluoride at high levels, suggesting that further research should be done.
